External. That's wild as I've never known anyone to have success with an external one. Very cool. And good luck!
2 years ago we did external ultrasound on 3 mares, all were pregnant. Our vet doesnt like to do inside and she's even a smaller lady.

It is unlikely that the foal will stay breech- just like human babies they flip around and if you are lucky you may even see junior do the "high dive" as it turns for the last time and enters it's forelegs into the passage- that is magic.

As to the teeth, if you can post a picture of the front teeth from the front and form the side I can tell you how old she is. It is virtually impossible to tell a four year old from a two year old as both have a "full" mouth, but it is dead easy to tell a three from a two and a three from a four, so I am not all that confident of your vet's diagnosis!
