EQUINE HEALTH RECORD

Hello,

My friend came me a nice copy of equine health record on her minis. I would like to know where I can down load this information. This papers has horse name, date foaled, sex, breed, dam, sire, reg, owner, address, city, state, zip, phone, email, web site,

along with vaccination history, deworming history, dental procedures, hoof care procedures, reproductive history, medical history.

I have been looking online,but have yet to found the one just like it. She does not remember where she got it.

and the jeffer's one's are cheap. I use them .... when I remember. I'm getting better though, it's hard for me since I'm such a forgetful person LOL!

I make my own. I designed what I wanted on MicroSoft Excel. It works pretty good and I don't have to buy them, just print them out.

Checked out the ones on the listed website, have to say that I'm not impressed. I prefer the ones I designed on Excel.
