Where are the AMHR national qualification forms located?

I can't find on their website where the national qualification forms are located. I called AMHR and most people had left early today and the lady I spoke with didn't know either. I need to print it out tonight so I can have it signed at a show tomorrow. Does anyone know?

In the member services tab click on member forms--then in show manager forms. Once you are there it is the last form, on page 2
