Anyone have the link to AMHR Nationals Info?

I just realized this morning that I am missing a page in my booklet due to the bad mailing system we have. Mine came to me in very bad condition

So I went to AMHR's website and they don't have it on there anymore! Does anyone have an link to get to it?

About 10AM today I was advised by AMHR office that during some updating in last couple of days......the entire listing of Nationals program has disappeared. They're working on getting it back up.
